Personal Care Services
If you’re facing challenges with daily living and are used to living independently, it’s important to know that support is available. There are services designed to help individuals maintain their independence while ensuring they receive the necessary assistance. The goal is to enhance quality of life and allow individuals to stay in the comfort and familiarity of their own homes. Whether you need a little help or more comprehensive support, these services can be tailored to fit your needs and preferences.

Consumer-Directed model
Member is responsible for:
- Hiring/termination of Caregiver.
- Agency responsible for Payroll, Billing, etc.

Eligibility
To qualify for our FREE personal care services:
- Consumers must have Full Medicaid coverage, Presbyterian, Blue Cross Blue Shield, Molina, United Healthcare.
- Turquoise Care
- Must be (21) years of age or older.
- Meet nursing facility level of care.
Consumer-Delegated model
Agency is responsible for:
- Hiring/termination of Caregiver.
- Scheduling.
- Payroll, Billing, etc.
Early and periodic screening, diagnosis, and treatment (EPSTD)
Personal care services (PCS) are a set of services that help children with disabilities receive assistance with daily tasks:
- Bathing: Help with bathing.
- Dressing: Help with dressing.
- Grooming: Help with grooming, including hair and nails.
- Toileting: Help with toileting activities.
- Eating: Help with eating and food preparation.
- Cognitive assistance: Provide cognitive assistance, such as prompting or cuing.
- Incidental household chores: Perform incidental household chores for the beneficiary.
- Accompanying to medical appointments: Accompany the beneficiary to medical appointments, but do not transport them.

Quality Care offers both Delegated and Directed Model Options:
- Consumers can choose either the Consumer-Directed or the Consumer-Delegated model.
- May pick their caregiver, which can be a family member or friend.
Consumer-Delegated model
Quality Care will also assist with::
- Developing an Individual Plan of Care with you.
- Monthly in-home visits by experienced Quality Care professionals and on-going caregiver supervision.
- Providing training for your Personal Care Attendant.
